Central Intake

Central Intake is an access and referral management service providing consumers/carers/patients with a single point of entry to those programs within the scope of Community Programs as well as facilitating navigation of the broader community service system.


  • To provide a single point of entry into Community Programs at Ballarat Health Services.
    Use common referral practices and protocols to facilitate transition between programs.
  • Support best practice privacy and confidentiality practices.
  • Provide easier, streamlined access to services for consumers, professionals and service providers.
  • Facilitate appropriate and timely referrals (internally and externally) and minimise duplication.
  • Ensure consumers and referrers receive a timely and appropriate response and outcome.
  • Maximise service delivery times for services.
  • Utilise the Service Coordination Tool Template (SCTT) and other appropriate tools to complete Initial Needs Identification.
  • Collect and collate data which can be used for mapping unmet needs, identifying service gaps, determining funding and service agreement requirements, and planning.
  • Be transparent and operate within a quality framework.
  • Work closely with community programs to maintain eligibility and priority of access criteria.
  • Be accessible and easy to negotiate for all client groups.


Seeking intervention from one of the following programs:

Post Acute Care; Hospital Admission Risk Program; Grampians Regional Continence Service; Cognitive, Dementia and Memory Service; Allied Health; Rehab in the Home; Grampians Regional Palliative Care Team; Planned Activity Groups; Linkages; Aged Care Assessment Service and Carer Respite and Support Services.

Eligibility for these programs is based on information provided to Central Intake.
