Cognitive, Dementia & Memory Services Clinic (CDAMS)

The Cognitive Assessment and Dementia Management Service (CDAMS) is a specialist diagnostic service for people experiencing memory loss or changes to their thinking and for those who care about them.

CDAMS provides expert clinical diagnosis, information and appropriate treatments, education, support, information and direction in future planning.

Early diagnosis is important to determine appropriate treatment needs and enable planning for the future and provides timely opportunity for clients and carers to learn about the condition, understand changes as they occur, and to cope with the day to day issues of cognitive impairment.

The initial assessment, whenever possible, is conducted in the client's home, and this is followed by a clinic appointment with a Geriatrician and may include neuropsychological and/or occupational therapy assessment.

At the conclusion of the assessment the outcomes and recommendations are given in writing to the client and their family or carer.

Monday to Friday 8:30am - 5pm

Outpatient/Referral Information

The preferred method of referral is through the client's GP, however they can be made directly by community agencies or through self referral.

If the referral is through a health care practioner, a copy of the Mini-Mental State Examination (MMSE), a copy of the blood test results and CT Brain report are requested.
