Community Rehabilitation, Community Health and Ambulatory Care

Community Rehabilitation

The community rehabilitation program provides a multidisciplinary rehabilitation service to enable clients who are disabled, frail, chronically ill or recovering from traumatic injury to achieve and retain optimal functional independence.
The program is delivered at the Ballarat Health Services Queen Elizabeth site. Clients may be treated on an individual basis or in small groups. Some group therapy programs have been devised for specific diagnostic groups such as Persistent Pain and Cardiac and Pulmonary Rehabilitation.

Community Health

The community health program provides services to improve the overall health of clients, particularly targeting individuals with chronic and complex health issues and aims to reduce the requirement for hospital and other specialist institutional services.
The program is delivered either at the Queen Elizabeth or Base Hospital sites, depending on the service required. Clients may be treated on an individual basis or in groups. Some therapy programs have been devised for specific groups, such as Healthy Weight Management.

VACS (Victorian Ambulatory Classification and Funding System)

The VACS program provides specialized Allied Health services to clients pre and post hospital admission.
The program is delivered either at the Queen Elizabeth or Base Hospital sites, depending on the service required and are generally provided on an individual basis.   

Waiting List

All referrals for group and individual therapies are to be sent to Central Intake where they will be placed on the appropriate waiting list. Waiting times for services vary and all referrals are prioritized using a common Allied Health Prioritization tool. Referrals are seen according to allocated priority.

Area Serviced

Allied Health Services are provided to clients in the Greater Ballarat region. Clients from the Grampians region can be seen when there is an identified need and transport can be arranged.

Fees

Fees vary according to the program/group and will be discussed with the client at the time a service commences.

Team of staff

A multidisciplinary team of Allied Health and Nursing Staff deliver services in the above programs. Staff working within the programs maintain professional responsibility to their discipline clinical manager in addition to program accountability.

The programs report to the Allied Health Ambulatory Care coordinator and through her to the Executive Director of Allied Health.

Referral

Referrals to this program are to be sent directly to Central Intake via RIMS (BHS - Community Programs - Central Intake) or alternatively faxed (using the SCTT templates) to 5320 3893. Clients can self refer via fax, phone or in person.
