Planned Activity Groups (PAGs)

Statement of Purpose

To engage people in meaningful group social activities which draw on personal strengths, build capacity, encourage independence and involve them in their community. The positive, supported environment provides social and respite opportunities for participants and their carers to prolong independent community living.

Our three PAGs were formerly known as Adult Day Centres.The programs are facilitated by staff with a minimum qualification of Certificate III Aged Care. Staff may provide limited assistance to individuals with general activities of daily living, such as eating, personal hygiene and mobility. Participants do need a minimal level of independence with mobility, and need to be able to cope within a group setting where the choice of activities may involve short excursions. The progress and care of all participants within our programs is planned and monitored with family, carers and supporting teams.

This Program is supported by financial assistance from the Commonwealth and Victorian Governments.

The PAG Unit:

Midlands Planned Activity Group

  • Monday to Friday 9.30am - 3.00pm for up to 28 participants each day. Funded by Home and Community Care (HACC)
  • Pool program weekly at Ballarat Aquatic & Lifestyle Centre

Elizabeth Brown Planned Activity Group

  • Monday to Friday 9.30am - 3.00pm for up to 15 participants. Funded by Home and Community Care (HAAC)
  • Saturdays 9.30am - 3.00pm for up to 10 participants. Funded by National Respite for Carers Program (NRCP)
  • A nine day program available during usual program closures over the Christmas - New Year season. Specific details available from October. Funded by National Respite for Carers Program (NRCP)
  • "Shed" program activities available on-site

Eyres House Planned Activity Group (Dementia Care)

  • Monday to Friday 10.00am - 3.30pm for up to 15 participants. Funded by Home and Community Care (HACC)
  • An overnight cottage respite program every second weekend for up to eight participants. Funded by National Respite for Carers Program (NRCP)
  • Younger Onset Dementia and Early Onset Dementia Programs available weekly

HAAC Funded Programs

Younger and older adults living in our community who may be frail or with special needs.

NRCP Funded Programs (Carer Respite)

Carers of younger and older adults living in our community who may be frail or with special needs.

Eyres House Planned Activity Group

Participants in the program must have a confirmed diagnosis of dementia.

Area Serviced

All Planned Activity Groups are located in the Ballarat region and service the Greater Ballarat Area.

Fees

Participants are charged a nominal fee to attend PAGs.This fee assists with basic daily programming costs, including morning and afternoon tea, the activity program and a two-course lunch.

Clients can participate in alternative activities scheduled each month at minimal additional cost.

Recipients of a Level 4 Home Care Package may be charged an alternative fee schedule based on an assessment of their needs against the eligibility criteria for the program.

Referral Process

Enquiries and referrals are welcome from the general public and health professionals. Referrals from health professionals are to be made using the Victorian Statewide Referral Service Co-ordination Template (SCTT) available on the Department of Health HACC website page.

All referrals are to be directed for processing to the Central Intake Team.

Team of Staff

The PAG unit has an overall manager, and each facility is staffed by a coordinator and program assistants.
