Post Acute Care

The Post Acute Care (PAC) program assists patients to recuperate following an acute hospital admission, sub acute admission or an emergency presentation and to facilitate their independence or transition to continuing care.

The program coordinates individually tailored packages of home based health and community care services via the provision of negotiation, purchasing and monitoring to aid discharge from hospital.

Eligibility

  • Patients who have been admitted to the Public Health Service and
  • A resident of Ballarat, Golden Plains, Hepburn or Moorabool Shires.

Referrals to this program are to be sent directly to Central Intake via RIMS (BHS - Community Programs - Central Intake) or alternatively faxed (using the SCTT templates) to 5320 3893. Clients can self refer via fax, phone or in person.
