Central Intake is the access and referral management service for all referrals received for community programs. Central Intake is the single point of entry to these.
What do we do?
Central Intake is for those requiring assistance from one of the following programs.
- Post Acute Care
- Hospital Admission Risk Program
- Grampians Regional Continence Service
- Cognitive, Dementia and Memory Service
- Allied Health Services
- Grampians Regional Palliative Care Team
- Planned Activity Groups
- Linkages
- Aged Care Assessment Service
- Carer Respite
- Support Services.
Eligibility for these programs is based on information provided to Central Intake.
Referrals
Referrals can be made by individuals, General Practitioners or from specialist services.
To place a referral through to Central Intake, please submit an e-referral or fax the referral form.
For a referral to be assessed, the following is required:
- Service Coordination Tool Template (SCTT)
- Victorian Statewide Referral Form (VSRF)
- Client Consent
