Grampians Watch supports people living with a range of health conditions to stay healthier at home.

Who is Grampians Watch for?

Any adult (18yrs+), living with complex medical conditions who may need support to get their health care needs met in the community, or at home, without needing to go to hospital. Grampians Watch is a Telehealth/Phone based Program delivery.                              

What happens if I join the Grampians Watch program?

You are allocated a Health Coach and a Telehealth Caller when you enroll and they remain with you on your journey, to ensure we get to know you and you us. This allows comfort in the fact you do not need to keep retelling your story and we understand your health needs and goals.

Your Telehealth Call Takers (non-clinical) create your support bookings and have regular phone calls scheduled with you to check in on your health. These calls are designed to spot health declines quickly, put measures in place to support you at home, and to help you avoid long wait presentations in hospital, if they are able to be resolved by your Health Coach/your GP or an after hour’s service provider.

Your Health Coach is a Clinical nurse, who is an expert in providing health information and advice about a range of medical conditions. Your Health Coach will listen to what’s important to you, and help you to get the health care you need at home.

How can Grampians Watch program help?

By staying in touch with you, the Grampians Watch team will check in with how you are feeling, and support you to get the help you need if you feel your health is getting worse.

Both your Telehealth Caller and your Health Coach allocated, can give you ongoing support whilst on the program to eventually promote confident independence at home.

  • An initial Health Assessment with a Health Coach is undertaken* to ensure a full conversation around your health goals is met
  • We can connect you with the services you need, or reconnect you with services that have been helpful in the past
  • We can provide referrals, including to Allied Health and support services (eg Counselling or Psychology, Podiatry, Physiotherapy or Drug and Alcohol services)
  • We can support you and your GP by letting them know how you are going, as well as supporting you to see your GP

FAQ

How much will this service cost?

Grampians Watch is a FREE program for you. Some resources and referrals may incur a cost that you would be made aware of beforehand.

How can I join the program?

Sometimes we find you!  We might contact you, if our Grampians Health team identify that you might benefit from the program. We will invite you to join the program, but it is your choice to participate.

You can refer yourself or a family member (with prior consent) by calling 1300 799 047.

Your GP can also refer you to Grampians Watch. Referrals can be sent to: grampianswatch@gh.org.au or fax (03) 5330 5156.
