What do we do?
The Complex Geriatric Clinic (CGC) offers comprehensive geriatric assessments to older people aged 65 years and above, with health or ageing related problems. These problems may include, but not limited to problems of cognitive or functional abilities, falls and balance, incontinence, frailty, medication-related and complex social situation.
What is required and how to proceed
As the clinic is a MBS-clinic, we require a referral from a GP or nurse practitioner (if applicable) detailing the older persons’ problem(s).

The Grampians Heath Ballarat Smart referral template can be used for CGC referrals or you may choose to address your referral to Complex Geriatric Clinic – including any one of the following Geriatricians: Dr Supriya Rama Krishnan, Dr Yasmin Sheikh, Dr Alfie Obieta, A/Prof Mark Yates.

Please attach the patient’s relevant investigation results (e.g. pathology results, brain imaging +/- cognitive test results) to the referral. Relevant past specialist(s) correspondence, may also be helpful. The clinic offers both Telehealth and face-to-face appointments.
